New Student Enrollment

  • ENROLLMENT PROCEDURES

    STEP 1

    Complete the REGISTRATION/ENROLLMENT REQUEST FOR RECORDS.

    STEP 2

    After completing the "REGISTRATION/ENROLLMENT REQUEST FOR RECORDS" email the form to enrollment@mjays.us.

    STEP 3

    Immunization records, birth certificates, proof of residency and parenting plans can be emailed to enrollment@mjays.us.

    • Accepted proof of residency includes utility bills and leases.

    • Immunization records will be requested from the previous school. In the event that the school does not supply the record, the parent will be called upon to do so. If your student has an immunization exemption card, the original MUST be turned in.

    • Kindergarten students are required to have all Kindergarten immunizations completed prior to the first day of school. If your student has an immunization exemption card, the original MUST be turned in. It is not to be uploaded.

    • 8th grade students must supply an immunization record that reflects the additional Tdap and MCV immunizations. You can email the student vaccination to enrollment@mjays.us. If your student has an immunization exemption card, the original MUST be turned in.

    • 12th grade students must supply an updated shot record that reflects the additional MCV immunization. You can email the student vaccination to enrollment@mjays.us. If your student has an immunization exemption card, the original MUST be turned in. It is not to be uploaded.

    STEP 4

    You will receive a link to complete Online Registration for your household.

    STEP 5

    When enrollment is complete, your student's building will contact you directly in regard to the first day of school.

    If you request transportation for your student during online registration, the driver will contact you directly in regard to times and locations of pick up. In some cases the driver may phone prior to enrollment being complete. Please wait for communication from the student's building to determine the first day of school for your student.
