2025-2026 Missouri Immunization Requirements

All children must provide proof of up-to-date immunizations, showing the month, day, and year for each vaccine before attending child care, preschool, or school.

Before the first day of school, please ensure your child meets the following requirements:

General Guidelines:

  • Vaccines may be administered up to 4 days early per ACIP guidelines.  It does not apply to the 28-day interval for live vaccines.
  • All immunizations must follow the Missouri DHSS (Department of Health and Senior Services)-recommended schedule and spacing.
  • Students who are “in progress” must have a completed Immunizations In Progress form on file and must not miss scheduled vaccine appointments.
  • Medical exemptions and religious exemption are accepted with completed forms on file. Parental exemptions are only for preschool and good for 1 year.  When going to school they need to be changed to medical or religious.  Those forms can be found on health.mo.gov
  • During outbreaks of vaccine-preventable diseases, unimmunized students will be excluded from school.
