Marshfield Athletics and Activities Registration

FinalForms 

We’re excited to announce that Marshfield Schools has partnered with FinalForms, a new online system for athletic and activity forms.

FinalForms makes it easy to complete, update, and sign participation forms for your student. Your information saves year-to-year, so you won't have to re-enter it each season!

Who needs to register?
All students planning to participate in sports or Marching Band must create a FinalForms account.

Getting started is easy:

  • Visit https://marshfield-mo.finalforms.com or scan the QR code below.

  • Follow the prompts to create your account, add your student(s), and complete your forms.

  • You’ll review and sign forms once per year or after any updates.

How to Create Your Account:

  1. Go to https://athens-oh.finalforms.com.

  2. Click New Account under the Parent icon.

  3. Enter your Name, Date of Birth, and Email, then click Register.

  4. Check your email and click Confirm Your Account.

    • Tip: If you don’t see the email within a few minutes, check your spam folder or contact support@finalforms.com.

  5. Create your password and confirm your account.

  6. You can choose to add another parent/guardian or skip this step.

  7. Once your account is created, click Register Student to add your child.


Registering a Student:

Information You’ll Need:

  • Medical history and health information

  • Doctor, dentist, and hospital contacts

  • Insurance company and policy number

  • Parent and student email addresses (if required)

Steps to Register Your Student:

  1. Log in at https://athens-oh.finalforms.com.

  2. Click Register Student.

  3. Complete the required information and click Create Student.

  4. Select the sport(s), club(s), or activity(ies) your student will participate in, then click Update.

  5. Complete and sign each form electronically.

  6. When finished, you’ll see a "Forms Finished" message.

    • Important: If required, your student will receive an email to sign their forms too.

You can log in at any time to update forms or add another student.
