We’re excited to announce that Marshfield Schools has partnered with FinalForms, a new online system for athletic and activity forms.
FinalForms makes it easy to complete, update, and sign participation forms for your student. Your information saves year-to-year, so you will not have to re-enter it each season.
Who Needs to Register?
All students planning to participate in sports or Marching Band must create a FinalForms account.
How to Get Started
Visit the FinalForms Registration Website:

Follow the prompts to create your account, add your student(s), and complete your forms.
You will review and sign forms once per year or after any updates.

How to Create Your Account

Go to the FinalForms Registration Website: https://marshfield-mo.finalforms.com

Select New Account under the Parent icon.

Enter your name, date of birth, and email, then select Register.

Check your email and select Confirm Your Account.

Tip: If you do not see the email within a few minutes, check your spam folder or contact support@finalforms.com.

Create your password and confirm your account.

You can add another parent or guardian, or choose to skip this step.

Once your account is created, select Register Student to add your child.
Registering a Student
Information You May Need

Medical history and health information
-
Doctor, dentist, and hospital contact information

Insurance company and policy number
-
Parent and student email addresses (if required)
Steps to Register Your Student

Log in at the FinalForms Registration Website: https://marshfield-mo.finalforms.com

Select Register Student.

Complete the required information and select Create Student.

Select the sports, clubs, or activities your student will participate in, then select Update.

Complete and sign each form electronically.

When finished, you will see a Forms Finished message.
Important:
If required, your student will receive an email prompting them to sign their forms as well.
You may log in at any time to update forms or add another student.
